Physical Therapist PT (Urgent Need)

Job Summary
Provide in-home physical therapy to patients, plan and administer prescribed PT treatments to attain highest level of physical function, assess and document patient strengths to guide treatment goals, develop and implement rehab programs aligned with physician plans, supervise and coordinate care with PTAs and Home Health Aides, document observations and progress, and report progress to physicians and care teams. Requires Doctorate/Professional degree in Physical Therapy, current license, and at least one year of PT experience; must have a valid state driver’s license and vehicle liability insurance; full-time role with pay-per-visit compensation details noted, and benefits described by Humana/CenterWell Home Health.
Required Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
- Experience collaborating with a team of healthcare professionals
- Driver's license and are expected to maintain personal vehicle liability insurance (Humana driver safety program)
