Job Summary
Deliver at-home physical therapy to patients, assess and document progress, and develop rehabilitation programs per physician Plan of Treatment. Supervise PTAs and Home Health Aides with clear instructions, coordinate with the care team, report patient progress to physicians and management, and ensure timely documentation including progress notes and discharge summaries.
Required Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
