Part-Time/Per Diem Home Health Physical Therapist (PT) needed in Taunton, MA. Responsibilities include evaluating patients’ conditions, developing and implementing individualized treatment plans, providing therapy per physician’s Plan of Care, supervising PTAs and HHAs, maintaining documentation and progress notes, educating patients and caregivers on exercises and equipment use, assessing home environments for modifications, collaborating with nurses and social workers, and submitting visit notes on time. Per-visit pay, mileage reimbursement, flexible scheduling, three weeks of PDO (for eligible employees), and one-on-one patient care are offered.
