Job Summary
Physical Therapist (PT) evaluates residents, develops individualized treatment plans, and provides skilled therapy to improve mobility, strength, balance, safety, and functional independence in a Skilled Nursing Facility. Works with physicians, nursing staff, and therapy team to maximize rehabilitation outcomes; maintains documentation and compliance with regulatory standards; participates in interdisciplinary care planning and family education; supports resident-centered care and ongoing professional development. Schedule includes Full-Time, Part-Time, and PRN opportunities; sign-on bonus for FT position.
Required Qualifications
- Current Colorado Physical Therapist (PT) license in good standing.
