Job Summary
Coordinate with a multidisciplinary team to individualize treatment plans for patients and their families in a supportive rehabilitation environment. Utilize progressive treatment modalities to address various diseases and impairments through in-house therapy with local management support. Ensure a comprehensive rehab experience while maintaining compliance with state licensing requirements and background check standards. Previous skilled nursing facility experience is preferred for this role within Liberty Healthcare and Rehabilitation Services.
Required Qualifications
- Must be a graduate of an accredited Therapy program
- Must be licensed in the state of the SNF you will be working
- Background checks/drug-free workplace
