Job Summary
Make home visits to clients in designated geographic territories across Western Cuyahoga County, performing diagnostic tests for mobility, balance, and strength. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while participating with nursing in multidisciplinary discussions. Educate clients and family members on rehabilitative care and instruct them on activities promoting independent living. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Maintain a current Ohio Physical Therapist license with at least one year of recent experience.
Required Qualifications
- A current OH Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills
Desired Qualifications
- prior experience with electronic medical records (EMR) preferred.
