Job Summary
Physical Therapist needed for full-time home health visits in senior living communities within New Castle County. Responsibilities include making home visits to clients in designated geographic territories, performing diagnostic tests and measurements (mobility, joint range, transfer status, balance, strength, endurance, gait), implementing protective wound care interventions, developing and updating individualized care plans, participating in multidisciplinary care planning with nursing, educating clients and families on rehabilitative care to promote health and independence, and accurately documenting observations and outcomes using an EMR/tablet. Qualifications require a DE Physical Therapist license, at least one year of PT experience, and graduation from an APTA/CAAHEA-approved program. Desirable skills include independent work, strong interpersonal and organizational abilities, problem solving, and comfort with EMR systems.
Required Qualifications
- A current Delaware Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
