Physical Therapist, (PT) - Senior Living Visits

Job Summary
Provide in-home Physical Therapy for adult and geriatric clients in senior living communities across Mecklenburg County, including evaluating mobility, gait, balance, and safety; developing and adjusting individualized care plans with manual therapy, gait training, and balance interventions; educating clients and families; documenting observations and assessments; and collaborating with nursing and multidisciplinary teams. Requires NC PT license, one year of PT experience, graduation from an approved program, ability to work independently, strong communication and organizational skills, and EMR familiarity. Field/home-health PT role with per-visit visits in designated geographic territories; emphasis on rehabilitation to enable independent living in home settings. Required Qualifications
- Current North Carolina Physical Therapist - PT license
- Minimum of one year of recent experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
