Job Summary
Conduct home health visits to adult and geriatric clients in Raleigh and Knightdale territories, performing diagnostic tests for mobility, balance, and muscle strength. Implement individualized care plans including manual therapeutic exercises, gait training, and integumentary protective interventions to prevent or repair wounds. Continually assess and revise care plans while collaborating with nursing staff, and educate clients and family members on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. Requires a current North Carolina Physical Therapist license, one year of recent work experience, and graduation from an accredited program.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current North Carolina Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
