Job Summary
Provide in-home physical therapy visits, including evaluations, reassessments, and fall risk assessments for seniors. Document accurately and communicate progress with the care team. Requires a current Hawaii license as PT or PTA, one or more years of clinical experience (home health preferred), and a valid driver's license with reliable transportation. Flexible per diem schedule allows you to choose your own patients. We are a growing Medicare-certified Home Health agency on O'ahu dedicated to fall prevention and helping seniors stay safe and independent at home.
Required Qualifications

- 1+ year of clinical experience
