Job Summary
Provide in-home physical therapy visits, including evaluations, reassessments, and fall risk assessments for seniors. Document progress accurately and communicate updates with the care team. Maintain a current Hawaii license as a PT or PTA with at least one year of clinical experience, preferably in home health. Possess strong documentation and communication skills, along with a valid driver's license and reliable transportation. Choose your own patients on a flexible or per diem schedule. We are a growing Medicare-certified agency dedicated to fall prevention and helping seniors stay safe and independent at home.
Required Qualifications
