Job Summary
Physical Therapist (PT) PRN position providing physical therapy services to residents across Sprenger Health Care’s senior living communities. Responsibilities include conducting resident screenings, evaluating data for treatment planning and implementation, coordinating treatment plans with residents, families, and the care team, and implementing prescribed therapies. Requires a Bachelor of Science Degree in Physical Therapy, successful completion of the National Certification Examination for Registered Physical Therapists, and current state licensure or licensure eligibility. Required Qualifications
- Bachelor of Science Degree in Physical Therapy from an accredited program
- Successful completion of National Certification Examination for Registered Physical Therapist
- Current state licensure or license eligible
- Provides physical therapy services to residents and initiates referrals as appropriate
- Conducts screenings of residents at regular intervals
- Evaluates residents to obtain data for treatment planning and implementation
- Coordinates treatment plan with resident, family and other team members
- Implements and conducts treatment as outlined in treatment plan
