Job Summary
Evaluate residents and develop individualized treatment plans in collaboration with families and the interdisciplinary care team. Provide physical therapy services focused on restoring mobility, strength, function, and independence. Conduct screenings to identify residents who would benefit from intervention, supervise PTA staff, and initiate referrals per department procedures. Document clearly and efficiently to support quality outcomes. This in-house role offers therapy time protection, clinical autonomy, and mentorship for new graduates within a family-owned network of 12 skilled nursing facilities.
Required Qualifications
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
- Current PT license or license-eligible
- Commitment to patient-first, outcomes-driven care
- Team-oriented mindset with strong communication skills
Desired Qualifications
- Experience working in SNF therapy under a contract model
