Job Summary
Physical Therapist (PT) PRN providing one-on-one geriatric home health care in West Palm Beach, Wellington, Greenacres, and surrounding areas. Role involves patient care in patients' homes, performing Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge), and implementing plans of care to improve mobility and safety. Collaboration with nursing/clinical leadership, with emphasis on efficient documentation, timely onboarding, and competitive per-visit rates under a 1099/PRN arrangement. Requires active FL PT license, CPR/BLS certification, valid driver's license and reliable transportation; Kinnser/WellSky experience is a plus. The position emphasizes fast onboarding, clinician-to-clinician support, and immediate case availability.
Required Qualifications
- Active Florida Physical Therapist (PT) License
- Current CPR/BLS Certification
- Valid Driver’s License and reliable transportation
- Kinnser/WellSky experience is a major plus
