Job Summary
Evaluate clients by obtaining and interpreting subjective and objective data to develop personalized treatment plans with short-term and long-term goals. Deliver high-quality care that empowers clients to achieve functional goals, perform re-evaluations to assess progress, and adjust care plans accordingly. Document all services accurately using Vivie's EMR system, complete supervisory visits with PTAs and HHAs in compliance with regulatory guidelines, and oversee continuation programs to optimize discharge outcomes. Maintain clear communication with clients, families, physicians, and the interdisciplinary team. Requires a current Minnesota Physical Therapist license, CPR certification within six months, and ability to physically perform duties. No travel required.
Required Qualifications
- Current Physical Therapist license in the state of Minnesota
- Strong collaboration and teamwork skills
- Clear and compassionate communication abilities
- Proficient in English for effective communication with staff, clients, and visitors
- Successful completion of a state-mandated background check
- Able to physically perform the duties required by the position
- Must obtain CPR certification within 6 months of hire
