Job Summary
Provide patient-centered physical therapy services across skilled nursing, long-term care, and home health environments. Responsibilities include evaluating, developing, and implementing individualized physical therapy treatment plans to improve mobility, strength, balance, and overall function; collaborating with nursing, physicians, and interdisciplinary team members to ensure continuity of care; educating patients and caregivers on treatment plans, safety, and home exercise programs; accurately documenting evaluations, treatments, progress notes, and discharge summaries; and ensuring compliance with regulatory and organizational standards.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in good standing (state-specific)
- Experience in skilled nursing, long-term care, or home health preferred (new graduates welcome to apply)
- Strong clinical, communication, and time-management skills
- Ability to work independently while contributing to a collaborative team environment
