Job Summary
Physical Therapist (PT) PRN in Fort Worth, TX, providing patient-centered therapy across skilled nursing, long-term care, and home health settings. Responsibilities include evaluating and implementing individualized PT treatment plans, improving mobility, strength, balance, and function, collaborating with nursing and physicians, educating patients and caregivers, and timely documentation of evaluations, treatments, progress notes, and discharge summaries while ensuring regulatory and organizational standards. The role offers a flexible part-time schedule and opportunities to work across multiple care settings within Pivot, with emphasis on independent work within a collaborative interdisciplinary team. Qualifications: graduate of an accredited Physical Therapy program and a current PT license in good standing; experience in SNF, LTC, or home health preferred; strong clinical, communication, and time-management skills; ability to work independently and as part of a team.
