Physical Therapist, PT – PRN

Provide physical therapy to adult and geriatric patients, prepare documentation including progress notes and discharge summaries, and establish a plan of care in accordance with patients' needs and capabilities. Travel to nearby centers as needed. Requires a valid State Physical Therapist License and credentials, with SNF and/or acute care experience. This PRN role supports Cornerstone Rehab's mission to meet and exceed patient outcomes across skilled nursing facilities in the United States.
