Job Summary
Conduct direct patient care, evaluate functional needs and outcomes, and adjust care plans to ensure optimal patient outcomes. Design and implement a plan of care based on thorough assessments, while directing support staff to achieve functional goals. Promote Elara Caring's philosophy and administrative policies to maintain quality of care, and provide direction and leadership to staff as a community resource. Document evaluations, treatment goals, and plans regularly, maintaining prescriptions and signatures within defined standards of care. Work in a physically demanding, high-stress environment involving lifting and transferring patients up to 50 pounds, with potential for irregular hours and 50% travel within the branch coverage area.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
