Job Summary

Perform comprehensive evaluations and develop individualized plans of care for residents, delivering skilled therapeutic interventions to restore function and independence. Monitor patient progress, adjust treatments based on clinical outcomes, and supervise Physical Therapist Assistants and support staff in accordance with state law. Communicate findings and discharge plans to physicians and families while instructing patients on home exercise programs and adaptive equipment. Participate in MDS assessments, care conferences, and quality assurance initiatives to ensure compliance with federal, state, and organizational standards. Maintain HIPAA, OSHA, and infection control protocols while treating all residents with dignity and compassion.

Required Qualifications

  • Current, active, and unrestricted Physical Therapist license in the State of California
  • Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate)
  • Successful completion of the National Physical Therapy Examination (NPTE)
  • Current CPR/BLS certification
  • Ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices
  • Ability to assist in resident evacuation during emergency situations
  • PRN arrangement availability
