Job Summary

Conduct comprehensive evaluations and develop individualized plans of care for residents in a skilled nursing setting. Deliver skilled therapeutic interventions, both individual and group, to restore function while monitoring progress and adjusting treatments based on clinical outcomes. Supervise Physical Therapist Assistants, rehabilitation aides, and volunteers in accordance with state regulations. Communicate evaluation findings and discharge outcomes to physicians and interdisciplinary teams. Develop and instruct patients and caregivers on home exercise programs and safe use of assistive devices. Complete timely documentation and participate in MDS assessments, care conferences, and quality assurance initiatives. Maintain compliance with HIPAA, OSHA, and California practice standards. Perform patient transfers and mobility support with full clinical autonomy.

Required Qualifications

  • Current, active, and unrestricted Physical Therapist license in the State of California
  • Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate)
  • Successful completion of the National Physical Therapy Examination (NPTE)
  • Current CPR/BLS certification
  • Ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices
  • Ability to assist in resident evacuation during emergency situations

Desired Qualifications

  • Experience in a skilled nursing, post-acute, hospital, or related medical setting
