Job Summary
Assess patient needs, develop recovery plans, and deliver physical therapy to help older adults stay active and thrive. Design individualized programs that restore, reinforce, and enhance physical abilities while training patients and caregivers on skills to promote independence. Collaborate with peers, make referrals to other care providers when necessary, and adapt to diverse patient needs across facility, outpatient, and home-based settings. Report to the Director of Rehab in a patient-centered environment focused on restoring hope and delivering personalized care exactly where needed.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
