Job Summary
Assess patient needs, develop recovery plans, and deliver physical therapy to help older adults stay active and thrive. Design individualized programs that restore, reinforce, and enhance physical abilities while training patients and caregivers on skills to promote independence. Collaborate with peers to address the whole patient, make referrals when necessary, and adapt care to diverse settings including skilled nursing centers, assisted living, outpatient clinics, and home-based care. Report to the Director of Rehab in a facility-based environment.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
