Physical Therapist (PT)

Conduct physical therapy evaluations and establish resident-centered treatment plans for strength, mobility, balance, and pain management. Deliver interventions including gait training, transfers, endurance strengthening, and balance exercises while monitoring resident response to modify plans. Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support care goals and maintain accurate, compliant documentation. Work within a collaborative rehab environment offering both post-acute and long-term care residents, supported by stable full-time scheduling and CEU reimbursement.
