Provide therapy services to patients in their homes based on the established plan of care, including evaluations, treatments, follow-up visits, and documentation. Collaborate with the clinical team, physicians, and caregivers to support patient goals and outcomes while educating families on therapy goals, safety, mobility, and activities of daily living. Maintain accurate and timely documentation in accordance with company, payer, and regulatory requirements and communicate patient updates to appropriate clinical team members. Current Florida license or certification for the applicable discipline is required, with prior home health experience preferred. Background screening is mandatory per Florida law.
