Job Summary
Physical Therapists (PT) and Physical Therapist Assistants (PTA) provide pediatric home-health therapy, implementing treatment programs to address physical, neurological, cognitive, and social-emotional needs in the home and community. Responsibilities include conducting evaluations, developing age-appropriate goals, creating and executing PT plans in collaboration with physicians, delivering skilled interventions to improve function, educating and empowering families to integrate therapeutic activities into daily routines, coordinating with referral partners, ensuring continuation of care post-discharge via home exercise programs, and thorough documentation in an EMR while maintaining patient confidentiality.
