Job Summary
Review patient charts and medical history, develop personalized treatment plans, and teach therapeutic exercises to improve patient conditions. Use hands-on therapy to promote healing while maintaining accurate records by tracking goals and adapting plans as needed. Work closely with physicians, nursing staff, social services, and home health aides to manage assigned caseloads and provide patient education for health promotion. Maintain the highest standard of care throughout all visits.
Required Qualifications
- New York State PT license
- Minimum of one year of recent hands on experience as a Physical Therapist
- Valid New York State Driver's License
- reliable transportation
- Work a minimum of 2 shifts per pay period (every 2 weeks)
- Attend the full first week of orientation, Monday through Friday from 8:30 AM – 4:30 PM
- If no prior CHHA experience, commit to training 2 days per week until fully oriented and independently managing
