Physical Therapist (PT) - Per-Diem

Travel to and from patients' homes to provide physical therapy as ordered by a physician, evaluating movement dysfunction through observation and listening to patient symptoms. Design personalized treatment plans and adjust interventions based on progress, utilizing hands-on therapy such as massage to ease pain and promote healing. Maintain patient records by tracking goals and adapting care, while instructing family members and caregivers in appropriate techniques. Ensure all services align with the designated plan of care and NY State licensure requirements.
