Job Summary
Follow physical therapy treatment plans for patients; establish, assess, and modify realistic, measurable, timely, and functional goals; oversee and evaluate care given by PTAs, Rehab Aides, and PT/PTA students; chart appropriately and timely; utilize therapy software; exhibit excellent customer service and a positive attitude towards patients; assist in the evacuation of patients; demonstrate dependable attendance; communicate and function on an interdisciplinary team; ability to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently; read, write, and understand English; Equal Opportunity Employer; Location: Payson, AZ.
Required Qualifications
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT)
- Currently registered/licensed in applicable State; active license in good standing
- CPR certification upon hire or during orientation; CPR certification must remain current
- One (1) year experience preferred; new grads welcome to apply
- Proficient in Microsoft Word, Excel, and email
- Liaisons with patients, families, support departments, etc., to plan for patient needs
- Knowledge of practices and procedures in post-acute care facility
- Ability to follow company policies including harassment and compliance procedures
- Exhibit integrity and professionalism per Life Care Code of Conduct
