Job Summary
Physical Therapist (PT) needed for part-time role in Olmsted Falls, OH area. Provides physical therapy services to residents, conducts screenings to determine intervention needs, evaluates residents for treatment planning, coordinates treatment plans with residents’ families and care teams, and implements treatments per plan. Requires a BS in Physical Therapy, successful completion of the National Certification Examination for Registered Physical Therapists, and current state licensure or licensure eligibility. Required Qualifications
- Bachelor of Science Degree in Physical Therapy from an accredited program
- Successful completion of National Certification Examination for Registered Physical Therapist
- Current state licensure or license eligible
- Provides physical therapy services to residents and initiates referrals as appropriate
- Conducts screenings of residents to determine need for intervention/treatment
- Evaluates residents to obtain data for treatment planning and implementation
- Coordinates treatment plan with resident, family and other team members
- Implements and conducts treatment as outlined in treatment plan
