Job Summary
Conduct comprehensive initial and ongoing assessments for home health patients, evaluating changes in status and determining necessary service modifications. Provide skilled interventions to help patients regain strength, manage pain, and achieve personalized goals in their daily lives. Educate patients, families, and caregivers on strategies to promote progress toward established treatment objectives. Submit detailed documentation of all treatments and interventions provided during visits. Minimum of five visits per week required. Compensation ranges from $95 to $110 per visit with weekly pay, travel time, and mileage reimbursement. Valid Arizona licensure, one year of PT experience, and a fingerprint clearance card are required.
Required Qualifications
- Current licensure or certification as a Physical Therapist active in Arizona
- One (1) year Physical Therapy experience
- Valid Arizona Fingerprint Clearance card
- CPR Certification (demonstration course required)
- Minimum requirement of 5 visits per week
- Must be available for weekend shifts
Desired Qualifications
- Experience working in home care setting preferred
