Job Summary
Complete initial and ongoing comprehensive assessments, evaluate patient status changes, and modify treatment plans as needed. Submit documentation of skilled interventions and educate patients, families, and caregivers to promote progress toward established goals. Provide one-to-one patient care in home settings across Green Valley and Sahuarita, Arizona, with a minimum of five visits per week at $95–$110 per visit. Flexible scheduling supports work-life balance, while weekly pay includes travel time and mileage reimbursement. New hires may qualify for a $1000 signing bonus after meeting specific hour or visit milestones.
Required Qualifications
- Current licensure or certification as a Physical Therapist active in Arizona
- One (1) year Physical Therapy experience
- Valid Arizona Fingerprint Clearance card
- CPR Certification (demonstration course required)
- Minimum requirement of 5 visits per week

- Experience working in home care setting preferred
