Job Summary
Provide physical therapy services in patients' homes within Lake County/Groveland and Leesburg, FL. Perform initial and ongoing assessments of mobility, coordination, strength, and function; develop and document a plan of care with goals and outcomes; deliver therapy per physician orders; maintain timely documentation; participate in discharge planning and educate patients and families on home exercise programs; collaborate with healthcare team and supervise applicable personnel (PTA, OTA, HHA) as needed; complete OASIS assessments where allowed by state practice; ensure adherence to policies, licensure regulations, and payer requirements; maintain effective communication across staff and family; position focuses on home health therapy delivery and outcomes.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
