Job Summary
Physical Therapist (PT) responsible for evaluating, planning, and providing skilled physical therapy services to residents to restore strength, mobility, balance, and functional independence. Conduct comprehensive evaluations and develop individualized treatment plans; provide gait training, strengthening, balance activities, pain management, and mobility training. Document evaluations, daily notes, progress reports, and discharge summaries per regulatory and facility standards. Collaborate with nursing, therapy team, physicians, and families; educate residents and caregivers on exercise programs and safety techniques; participate in interdisciplinary care conferences and contribute to care planning. Maintain compliance with state, federal, and professional practice regulations.
Required Qualifications
- Current Physical Therapist (PT) license in the state of California
- Graduate of an accredited Physical Therapy program
