Job Summary
Evaluates and conducts medically prescribed physical therapy treatment programs to treat movement dysfunction and pain resulting from injury, disease, disability, or other health-related conditions. Establishes, revises and evaluates a plan of care with patient/family; performs therapy interventions using standard PT techniques; provides education to patient/family/staff; participates in discharge planning and follow-up care; oversees physical therapy assistants and students; maintains documentation; and contributes to home instructions and equipment/home modification recommendations. Requires licensure as a Physical Therapist in the practicing states and BLS HCP certification within 90 days; typically a day shift with 32 scheduled hours per week in the East PT department.
Required Qualifications
- Licensed Physical Therapist in Illinois (IDFPR) OR Licensed Physical Therapist in Wisconsin (DPSD) OR Licensed Physical Therapist in Missouri (DPR) OR Licensed Physical Therapist in Oklahoma (Medical Board)
- BLS Healthcare Provider certification within 90 days of employment
- Bachelor's, Master's, or Doctorate degree in Physical Therapy or related field as required to obtain licensure
- No experience required
