Physical Therapist (PT), Outpatient

Evaluate, plan, and deliver evidence-based physical therapy interventions to patients across acute hospital, acute rehabilitation, inpatient, medical-surgical, and outpatient care settings. Conduct comprehensive assessments, develop individualized treatment plans, and provide therapeutic interventions aimed at improving mobility, strength, pain management, and overall functional independence. Collaborate closely with the interdisciplinary care team, participate in discharge planning, and educate patients and caregivers on home exercise programs and safety strategies. Ensure accurate, timely documentation and maintain compliance with regulatory and facility standards while delivering high-quality, patient-centered care. Additional duties may be assigned as needed.
