Job Summary

Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations to establish and implement individualized plans of care. Monitor patient outcomes, adjust treatment based on response, and manage caseloads while supervising physical therapy assistants, extenders, or students as needed. Coordinate patient discharge by providing home programs, teaching families, and ordering equipment, while collaborating with stakeholders to ensure continuity. Complete daily documentation, statistics, billing, and scheduling in adherence to insurance guidelines and accreditation standards. Maintain a clean treatment environment, perform manual techniques and patient transfers, and participate in safety initiatives and performance improvement activities. Work 40 hours per week with a $5,000 sign-on bonus and a pay range of $41.10 to $61.65.

Required Qualifications

  • Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation
  • Licensed Physical Therapist in the State upon which you work
  • Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
  • Must be able to lift up to 50 lbs. from floor to waist
  • Must be able to lift up to 10 lbs. from waist to overhead
  • Must be able to lift and carry up to 40 lbs. at waist height a reasonable distance
  • Must be able to push/pull with 30 lbs. of force
  • Must be able to perform a sliding transfer of 150 lbs. with a second person present
  • Must have functional use of senses to allow for effective communication
  • Must be able to wear protective clothing as needed

  • Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs
