Job Summary
Evaluate and treat older adults in their private residences or independent living facilities, developing and implementing individualized physical therapy programs to restore function and promote independence. Conduct comprehensive screenings, history reviews, and functional assessments to establish diagnoses, set goals, and design treatment plans including therapeutic exercise, manual therapy, gait training, and falls prevention strategies. Instruct patients and caregivers on mobility skills, adaptive device usage, and chronic disease management while making referrals to other disciplines for comprehensive care. Supervise physical therapy assistants and support personnel, ensure timely clinical documentation and billing compliance, and maintain equipment safety standards. Maintain a valid Physical Therapy license and provide transportation to patient locations.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
