Job Summary
Deliver quality home healthcare to patients of all ages in Ramona and surrounding areas using the Cliniconnects app to accept or decline referrals. Observe, record, and report patient responses to treatment while determining the plan of care. Maintain accurate documentation within our intuitive Electronic Medical Record system, often completing tasks during the visit. Work as an independent contractor with full control over your schedule, coverage areas, and income based on per-visit rates. Reliable transportation and a smartphone are required; current, unrestricted state licensure is mandatory. Join a network connecting clinicians to over 900 home health agencies to improve patient outcomes by removing administrative burdens.
Required Qualifications
- Ability and enthusiasm in working with all ages
- Current Physical Therapist license issued by the state Board of Physical Therapy
- The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
- Completion of an accredited Physical Therapy program
- Reliable personal transportation
- Smartphone
