Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, evaluating and treating patients in their private residences or independent living facility rooms. Develop and deliver individualized physical therapy programs to restore function, instruct patients and caregivers in essential skills, and make referrals to other disciplines for comprehensive care. Screen, examine, and evaluate patients to establish diagnoses, design treatment plans incorporating therapeutic exercise, manual therapy, and adaptive devices, while adapting protocols based on individual progress. Supervise Physical Therapy Assistants and students, ensure adherence to clinical and billing documentation requirements, and maintain a safe environment by identifying and reporting equipment needs.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
