Job Summary
Evaluate and treat older adults in private residences and independent living facilities by screening, examining, and establishing diagnoses. Develop and implement individualized physical therapy programs including therapeutic exercise, manual therapy, gait training, and adaptive device fabrication. Instruct patients and caregivers in mobility skills, conduct falls assessments, and make referrals to other disciplines for comprehensive care. Supervise Physical Therapy Assistants, students, and support personnel while adhering to state licensure requirements and documentation standards. Maintain equipment, ensure safety compliance, and participate in staffing conferences. Requires a valid PT license, Master's degree, and reliable transportation.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
