Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, evaluating, treating, and developing physical therapy programs to facilitate patient independence. Screen, examine, and evaluate patients to establish diagnoses, identify impairments, and design individualized treatment plans including therapeutic exercise, manual therapy, and functional training. Instruct patients and caregivers in essential skills while directing activities to promote health and safety, and make referrals to other disciplines to address comprehensive patient needs. Supervise Physical Therapy Assistants and support personnel as required, maintain accurate clinical and billing documentation, and adapt care plans based on patient progress.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
