Job Summary
Evaluate and treat older adults in home and independent living settings, delivering individualized physical therapy programs to restore function and promote independence. Conduct screenings, examinations, and mobility assessments to establish diagnoses and develop treatment plans, while instructing patients and caregivers in therapeutic techniques and adaptive device use. Manage referrals to other disciplines, supervise assistants and students, and maintain accurate clinical and billing documentation. Ensure patient safety by enforcing hazard elimination and maintaining equipment, all within a flexible schedule.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
