Job Summary
Evaluate and treat older adults in home and independent living facility settings by developing and delivering individualized physical therapy programs. Conduct screenings, examinations, and mobility assessments to establish diagnoses, identify impairments, and design treatment plans incorporating therapeutic exercise, manual therapy, and functional training. Instruct patients and caregivers on skills for independence while managing referrals and adapting plans based on patient progress. Supervise therapy assistants and support personnel, ensuring adherence to documentation requirements and safety standards. Maintain equipment and order supplies as needed.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
