Job Summary
Evaluate and treat older adults in their private residences or independent living facilities by developing and delivering individualized physical therapy programs. Conduct comprehensive screenings, examinations, and mobility assessments to establish diagnoses and design treatment plans covering therapeutic exercise, manual therapy, functional training, and adaptive device application. Instruct patients and caregivers on skills essential for promoting independence, while supervising Physical Therapy Assistants and support personnel as applicable. Adhere to department schedules, ensure timely clinical and billing documentation, and make referrals to other disciplines to address comprehensive patient needs. Maintain a safe environment by enforcing hazard elimination, ordering necessary supplies, and reporting equipment repairs.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
