Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, evaluating and treating patients to facilitate restoration of function. Develop and implement individualized physical therapy programs designed to enhance performance while instructing patients and caregivers in skills essential for promoting independence. Screen, examine, and evaluate patients to establish diagnoses, design treatment plans, and complete related documentation and record keeping. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements. Adapt plans of care according to individual patient needs and consult with the rehabilitation team to select appropriate therapeutic regimens. Order supplies, maintain equipment, and perform home and community mobility assessments to ensure safe environments.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
