Job Summary
Evaluate, diagnose, and treat older adults in their private residences or independent living facilities through a comprehensive physical therapy program. Deliver therapeutic exercise, manual therapy, and functional training to restore mobility and independence while instructing patients and caregivers on chronic disease management and fall prevention. Collaborate with the rehabilitation team to design individualized plans, order adaptive devices, and make referrals to other disciplines as needed. Supervise PTAs, students, and support personnel in accordance with state licensure requirements. Maintain accurate clinical and billing documentation, attend staffing conferences, and ensure a safe environment by identifying and reporting equipment hazards.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
