Physical Therapist (PT) - Mobile Outpatient

Evaluate and treat older adults in private residences and independent living facilities, delivering personalized physical therapy programs to restore function and promote independence. Conduct screenings, examinations, and mobility assessments to establish diagnoses, develop treatment goals, and implement therapeutic techniques including exercise, manual therapy, and adaptive device training. Instruct patients and caregivers on skills for safe mobility and chronic disease management while making referrals to other disciplines for comprehensive care. Supervise Physical Therapy Assistants, students, and support personnel as needed, ensuring adherence to clinical documentation, safety standards, and department schedules. Maintain equipment, order supplies, and participate in staffing and care conferences.
