Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, evaluating, treating, and developing individualized physical therapy programs to restore function and promote independence. Conduct screenings, examinations, and functional assessments to establish diagnoses, design treatment plans, and implement therapeutic techniques including exercise, manual therapy, and mobility training. Instruct patients and caregivers on skills essential for safe mobility, manage referrals to other disciplines, and adapt plans based on patient progress. Ensure adherence to clinical documentation, safety standards, and department schedules while supervising support personnel as needed.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
