Job Summary
Evaluate and treat older adults in home and independent living facility settings, developing and implementing individualized physical therapy programs to restore function and promote independence. Conduct comprehensive screenings, examinations, and mobility assessments to establish diagnoses, identify impairments, and design treatment plans including therapeutic exercise, manual therapy, and adaptive device training. Instruct patients and caregivers in essential skills while making referrals to other disciplines and consulting the rehabilitation team to address comprehensive patient needs. Supervise Physical Therapy Assistants, students, and support personnel as required, ensuring adherence to clinical documentation, safety protocols, and department schedules.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
